                                          ************************************************************************

Commencing August 2, 2011, I agree to make a payment of $    no later than Tuesday of each week for a period of 20 weeks. I understand that if a payment is delinquent I will not be permitted to attend classes the following week, and that I will continue to be denied attendance privileges until all payments are current. My absence from classes will incur a zero for the class work on those days, and missing more than the number of days than course credits for the individual courses will lower the final grade in those classes. After September 2, 2011, I am responsible to pay all the monies due, even if I do not finish the semester.
